Requirements

According to Federal and State Regulations, certain documents must be presented to MAC in order to open a business account. Here’s what you’ll need to get started.

Military FRG and Company Accounts

• Letter from the Commander to establish or  make any changes to the account
• List of names to be added or removed and specific changes of address or services
• EIN Federal Tax Number (To get your EIN number, click here to go to www.irs.gov)

Sole Proprietor

• Official Business License.
• Certificate of Assumed Name (only if your full name is not included in the business name)
• Social Security Number or EIN Federal Tax Number (To get your EIN number, click here to go to www.irs.gov)

Corporation

• Official Business License.
• Articles of Incorporation (filed with state)
• Corporate Resolution for Signers
• EIN Federal Tax Number (To get your EIN number, click here to go to www.irs.gov)

General Partnership

• Official Business License.
• Copy of Partnership Agreement
• EIN Federal Tax Number (To get your EIN number, click here to go to www.irs.gov)
• Certificate of Assumed Name

• Partnership Resolution for Signers
